Sick note for self isolating

If you have to self isolate during the Covid Pandemic you can obtain a sick note by going onto the following site: 111.nhs.uk/isolation-note/.

Please ensure your GP surgery also has your email address.

You can do this by sending a message using SystmOnline if you have access or calling the Surgery.
