The GP Patient Survey lets you have the opportunity to give us your views on your local health services. Some patients registered here, at Dr I.M. Raja & Partner, will soon receive a questionnaire asking you about your experiences.
The survey covers issues that concern patients, such as access to care and your satisfaction with your treatment.
As a Surgery, we do not know who will receive this survey but we would ask that if you do receive one, please complete it and send it back as soon as you can.
It is so important that we hear your views; even if you have received one before, please fill it in and return it.
Your response will help us to improve the services we provide for you and your fellow patients.
